Long Term Conditions clinics- Appointment Invitation Service

We are trialling a new way of booking in for your Long Term Condition Reviews. You may receive a text message from us with a link that takes you directly to our Appointment Invitation Service – you will be asked to input your date of birth and will then have the option of booking directly into one of our clinics (some of you may have used the service in the past for arranging your flu/ covid vaccine appointments). You can still contact the Surgery to arrange your appointment so please don’t worry if your phone does not have internet facilities.
