Anyone in England can register with a GP surgery.

You do not need proof of address or immigration status, ID or an NHS number.

Pre-registration with the practice

We will soon have an online form available for you to pre-register with your details. You will then need to come to the surgery to sign the form to confirm that the details are correct and provide ID.

You can now register with our practice simply and quickly online.

By hand

Please download and print this form, fill it in, and bring it in with you. We have copies available at our reception desk if you do not have a printer.

If you need help completing the form please speak to our reception team who can assist you.

You do not need proof of ID to register with a GP, but it might help if you have one or more of the following:

  • passport
  • birth certificate
  • HC2 certificate
  • rough sleepers’ identity badge
  • hostel or accommodation registration or mail forwarding letter

If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.
